Taylor Swift turned the 2026 MTV Video Music Awards into the premiere stage for “Patient Zero,” a dark new music video starring Dakota Johnson and Colin Farrell. Cara Delevingne and Los Angeles Rams quarterback Matthew Stafford also appear in the visual, which Swift directed with Oscar-winning cinematographer Emmanuel Lubezki behind the camera.
The video casts Swift as the ghost of Farrell’s former wife, who attempts to warn Johnson’s character about her husband. The gothic story draws on ghost tales and psychological thrillers, with Delevingne appearing as another woman in Farrell’s character’s life. Swift had kept the project secret for months before its world premiere at the VMAs.
The premiere came during a major night for the “Cruel Summer” singer at the Peacock Theater in Los Angeles. She received the inaugural MTV VMA Artist Director Honors, a new award created to recognize artists who have expanded music-video storytelling through their work behind the camera. The Grammy winner used her acceptance speech to reflect on two decades of making music videos. She said she has participated in roughly 60 videos and written and directed 18 of them. Dakota Johnson presented the award before Swift introduced “Patient Zero” to the audience.
Swift also took home Video of the Year for “The Fate of Ophelia” and Best Direction for “Opalite.” The wins pushed her past Beyoncé’s previous record and gave the singer-songwriter a new career high at the VMAs. She entered the ceremony with 30 Moon Person wins and 11 nominations.
“Patient Zero” comes from “The Life of a Showgirl: The Encore,” which arrived September 25 with four new songs. On Spotify, the track recorded about 13.7 million global streams in its first day, becoming the platform’s most-streamed song by a female artist in a single day in 2026. Swift herself also became Spotify’s most-streamed female artist in a single day this year.
